It is the primary physiological inhibitor of the fibrinolytic system, negatively regulating fibrin degradation by rapidly inhibiting tissue-type plasminogen activator (tPA) and urokinase-type plasminogen activator (uPA). This central role enables PAI-1 to critically influence thrombus formation, tissue repair, and fibrosis. The expression of PAI-1 is induced by various signaling stimuli, and its elevated levels are closely associated with thrombotic diseases, metabolic syndrome, organ fibrosis, and cancer progression. Furthermore, PAI-1 serves as a marker of the senescence-associated secretory phenotype (SASP) and is involved in the process of cellular senescence.
