Background
B and T lymphocyte attenuator (BTLA), also known as CD272, is a type 1 transmembrane glycoprotein and is a member of the CD28 immunoglobulin (Ig) superfamily. BTLA has an extracellular IgV domain, and the cytoplasmic domain is made up of a growth factor receptor-bound protein 2 (GRB2) association motif, an immunoreceptor tyrosine-based inhibitory motif (ITIM), and an immunoreceptor tyrosine-based switch motif (ITSM) (1-3). BTLA is also expressed in a soluble form (sBTLA), produced as a result of alternative splicing (4). BTLA is mainly expressed on B and T cells and has been observed on murine type 1 conventional dendritic cells (cDC1) (5). BTLA interacts with its ligand Herpesvirus entry mediator (HVEM), a member of the TNF receptor superfamily, which induces phosphorylation of ITIMs and Src homology 2 (SH2) domain-containing phosphatase 1 (SHP-1)/SHP-2 association leading to repression of T cell proliferation and cytokine production (6,7). GRB2 binding to the GRB2 association motif leads to the recruitment of PI3K protein subunit p85 and T cell activation (1). BTLA expression is one of the markers used to help identify exhausted and dysfunctional lymphocytes and often correlates with cancer progression (8). Elevated sBTLA plasma levels are associated with sepsis, autoimmune disease, and cancer (9,10). The BTLA-HVEM axis is an important immune checkpoint in CAR-T cell immunotherapy, and strategies to overcome this hurdle are under investigation (11).
