Background Insulin-degrading enzyme (IDE) is a highly conserved zinc metalloprotease responsible for clearing short bioactive peptides, including insulin, glucagon, amylin, and β-amyloid (Aβ). IDE has the highest affinity for insulin and plays a critical role in regulating insulin levels across various tissues in the body (1). The enzyme contains two domains, IDE-N and IDE-C, which together form a dynamic catalytic chamber essential for substrate recognition, selectivity, and processing (2,3). IDE deficiency or dysfunction has been linked to type 2 diabetes mellitus and hyperinsulinemia due to impaired insulin clearance, making IDE a potential therapeutic target for diabetes (4). IDE has also been implicated in AD pathogenesis due to substrate competition between insulin and Aβ, and may provide insight into the connection between diabetes and Alzheimer's disease (AD) (5).
