The Sodium Iodide Symporter also called NIS or SLC5A5 actively transports iodide into cells particularly in the thyroid gland. This protein weighs approximately 87 kDa and primarily locates in the thyroid follicular cells. It exists also in lactating mammary glands salivary glands and gastric mucosa. By concentrating iodide in these tissues NIS plays an important role in synthesizing thyroid hormones which are essential for metabolism regulation.
Biological function summary
NIS facilitates iodide uptake into thyroid cells enabling the synthesis of thyroid hormones triiodothyronine (T3) and thyroxine (T4). This protein doesn't work alone; instead it is a part of a larger thyroid hormone synthesis complex interacting with thyroid peroxidase (TPO) and thyroglobulin (TG) to ensure efficient hormone production. Its action depends on the sodium gradient established by the Na+/K+ ATPase pump which drives active transport against the concentration gradient.
Pathways
NIS forms a critical component in the hypothalamic-pituitary-thyroid (HPT) axis essential for maintaining the body's metabolism and energy balance. This axis forms a feedback loop where thyrotropin-releasing hormone (TRH) and thyroid-stimulating hormone (TSH) regulate the expression and activity of NIS. In the iodine metabolism pathway NIS cooperates with pendrin a chloride/iodide transporter establishing a seamless flow of iodide into thyroid follicles for hormone synthesis.
NIS dysfunction relates to thyroid-related conditions such as congenital hypothyroidism and papillary thyroid cancer. In congenital hypothyroidism NIS mutations reduce iodide uptake leading to insufficient thyroid hormone production. Papillary thyroid cancer might exploit NIS for its malignancy but altered regulation hampers effective radioiodine therapy. In these disorders the interplay with proteins like TPO and TG alters the normal hormone synthesis process causing significant health issues.
