Background Pomalidomide, a derivative of thalidomide, is an immunomodulatory drug (IMiD) that binds to the CRL4 E3 ubiquitin ligase substrate receptor, cereblon (CRBN), leading to the targeted degradation of transcription factors Ikaros and Aiolos (1,2). The degradation of these transcription factors initiates several downstream responses that play a role in pomalidomide's antitumor and immunomodulatory properties (3). Pomalidomide enhances T cell and natural killer (NK) cell-mediated responses, resulting in potent anti-inflammatory and anti-tumor effects while inhibiting the expression of pro-inflammatory cytokines, such as tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6) (3,4). Pomalidomide is FDA-approved and widely used in combination therapies for multiple myeloma (MM), particularly in patients who have relapsed or become refractory to other treatments (4). When taken in combination with low-dose dexamethasone, pomalidomide has demonstrated efficacy in reducing disease progression and improving survival rates for patients with MM (4).
