ITIH4 also known as inter-alpha-trypsin inhibitor heavy chain H4 is a serum protein with a molecular mass of approximately 120 kDa. It belongs to the inter-alpha-trypsin inhibitor (ITI) family. This protein is predominantly expressed in the liver and circulates in the blood. ITIH4 operates mechanically by binding to hyaluronic acid giving it a protective capability in the stabilization of the extracellular matrix.
Biological function summary
ITIH4 contributes to the inflammatory response and tissue repair processes. It forms part of a protease inhibitor complex known as ITI comprised of heavy and light chains that interact together. This complex acts in protecting tissues from damage by inhibiting serine proteases. Additionally ITIH4 has adaptive roles in modulating the inflammatory response facilitating tissue restoration and engaging in immune cell regulation.

ITIH4 associates with inflammation and extracellular matrix organization pathways. Inflammation-related pathways involve interactions with several cytokines and matrix metalloproteinases contributing to the regulation of inflammatory responses. Within these pathways ITIH4 exhibits connection with proteins such as interleukin 6 (IL-6) playing roles in inflammation and tissue communication.
ITIH4 has been implicated in liver diseases and certain cancers. Its altered expression and functionality have associations with liver fibrosis where it may influence the extracellular matrix dynamics and hepatocellular carcinoma potentially impacting tumor microenvironments. ITIH4's connections to liver disease also relate it to alpha-1 antitrypsin which plays a part in liver protection and disease modulation.
