Background Agrin is an extracellular matrix heparan sulfate proteoglycan. Alternative splicing within the C-terminus of the protein generates neural and non-neural agrin, which interact with distinct binding partners to engage different signaling cascades (1). Neural agrin plays a crucial role in the formation of skeletal neuromuscular synapses between presynaptic motor neurons and postsynaptic muscle fibers, as well as in synaptic differentiation (2,3), through its interaction with the transmembrane protein LRP4, which leads to acetylcholine receptor (AChR) aggregation via the activation of MuSK (4,5). Agrin binds to α-dystroglycan (DAG1) and promotes cardiac regeneration (6,7). Agrin is upregulated in non-small cell lung cancer, hepatocellular carcinoma, and pancreatic ductal adenocarcinoma. It promotes tumor angiogenesis, EMT, and metastasis by maintaining VEGFR2 stability on the cell membrane, regulating focal adhesions, and activating PI3K/Akt and YAP pathways (8-12).
