Background Follistatin-related protein 1 (FSTL1, TSC-36, or FRP) is a secreted glycoprotein that is a member of the SPARC family (1). FSTL1 modulates multiple biological processes through various signaling pathways. FSTL1 acts as an antagonist of BMP4 to regulate the TGF-β/BMP/Smad signaling cascade (2), which is required for lung, smooth muscle, and skeletal development (2-4). As a ligand of the receptor DIP2A, FSTL1 activates downstream Akt/eNOS signaling (5), which protects the heart during injury and promotes tissue regeneration during ischemia stress and cardiovascular disease progression (6,7). FSTL1 is involved in the activation of several additional signaling cascades, such as NF-κB, Erk, AMPK, and Wnt/β-Catenin pathways (8,9).
